If a company offers a vision plan, employees will be brought to the vision plan selection page after they've enrolled in or declined a medical plan and dental plan.
You can check the box for each dependent you would like to enroll to view the plan rates. In order to enroll a dependent, you must also enroll in vision coverage.

- Monthly Cost
- Frame allowance
- Lens allowance
- Contacts allowance
If you would like to decline coverage, you can do so by clicking on the "I decline to enroll in vision insurance" link at the bottom of the page.
If a Medical plan with bundled Vision coverage was selected, that Vision plan will be automatically selected.
