Benefits Reports

The Benefits Administration app contains several useful reports you may need to assist in administering your company's benefits. Below, you'll find a detailed breakdown of what you can expect from each report available.


Benefits Quoting Census Report

Information about current employee and dependents, demographics, enrollments, plan and cost that is required for quoting.
  • Last name, first name, employee ID (Zenefits ID), dependent type
    --dependent type will list employee, child, or spouse if the employee has dependents enrolled
  • Gender, date of birth, age, hire date, job title, hours worked per week, hourly/salary pay, annual salary, SSN, tobacco usage (Y/N), disabled (Y/N), marital status, phone number, email, address, city, state, zip code
  • Line of coverage (medical, dental, or vision), enrollment complete (Y/N), enrollment status, effective date, carrier, plan type, plan name, decline reason, total monthly cost, monthly deduction, monthly company contribution, elected amount (basic), elected amount (voluntary), approved amount (basic) approved amount (voluntary)

Benefits Audit Report

Includes deduction and contribution information for all lines of coverage
  • Last name, first name, employee ID (Zenefits ID), dates, employment status, termination date
  • Line of coverage (medical, dental, or vision) --will list the benefits event (open enrollment, new hire enrollment, etc.) under the month that the event occurred), plan name, coverage tier, employee deduction, company contribution, total premium
    -- this information repeats for every line of coverage

Benefits Cost Report

Includes Excel spreadsheets with selected deduction and contribution information for all lines of coverage
  • Last name, first name, employee ID (Zenefits ID), line of coverage (medical, dental, or vision)
  • Deductions tab: pay frequency, current pay period, check date, current deduction, normal deduction, deduction adjustments, benefits event (open enrollment, new hire enrollment, etc.), effective date of benefits event, end date of benefits event
  • Contributions tab: pay frequency, current pay period, check date, current contribution, normal contribution, deduction adjustments, benefits events, effective date of benefits event, end date of benefits event
  • Requested Deductions tab: reason code, effective date, creation date, deduction, contribution, prorating strategy, opted into pay stubs
  • Paystub Deductions tab: employee amount, employee amount YTD, company amount, company amount YTD, start date, end date, pay date

Post Open Enrollment Reports

There are two reports to assist with submitting employee enrollments after open enrollment has ended:
  • Renewal Enrollment Report: List of all employees and plans
  • Renewal Change Report: Lists only employees who select a new plan or a plan that was not mapped to their previous plan
Reports can be downloaded either from the Benefits Administration app, or specific to a line of coverage through Fulfillment Tasks.
